I actually just researched your question lol. Well, during your consultation, the surgeons will most likely consider your emotional and physical maturity. If your face is still going to grow, then they might delay surgery for a future date (for obvious reasons). I have no idea how they would examine emotional maturity, but I suppose the person who uploaded this video would have more information.

I think (key word: think) you have to be 16 or over, unless you have a parent's permission. But it depends where you live.

Ha! I'll consider it a compliment that some people suppose that her "after" picture is photoshopped. It is, in fact, her actual post-op photo...un-manipulated, un-photoshopped.
